Customer Registration
Dear Customer.
This service is free. By registering with us we will dispense your prescriptions for Medical Appliances (not drugs) promptly, discreetly, and free of charge at a time, place, and location of your choice. We will also give you free dry/wet wipes and disposal bags with each prescription we dispense – you choose.
You can register with one of the 2 online forms below or print off the registration form by clicking on the ‘Download’ button below. Once filled in you can post it to us – call our helpline for a supply of our Freepost envelopes.
- Quick Registration Form: We ask you for the basic minimum information to get you started using our service.
- Full Registration Form: There are a lot of questions on the full registration form but please don’t let this deter you from filling it in, as the more information you can provide the better the service we can tailor to suit your specific needs.
Your personal information is safe with us, we will never pass it on to anyone else and that is a promise! We will always call you after receiving your registration to introduce ourselves so you have a named personal helpline adviser not just someone from a call centre. Your medical devices will be dispatched promptly, packaged discreetly, and delivered at a time, place and location of your choice. We can phone/email/SMS Text you to inform you what day and give you an estimated time slot when you’re consignment will be delivered or you can track your packages progress online via email and/or receive SMS Text Messages to your Smartphone. You can even alter the location and time of delivery within certain parameters once the package has been dispatched.
